About the position

The Clinical Evaluator (PASRR) position at Acentra Health involves conducting face-to-face assessments for individuals with serious mental illness, intellectual disabilities, or related conditions. The role is crucial for ensuring appropriate placement and care for patients entering nursing homes, enhancing their well-being and quality of life. This hybrid position requires strong clinical assessment skills and the ability to work collaboratively with healthcare professionals and families.

Responsibilities

  • Conduct face-to-face clinical assessments for individuals with serious mental illness and intellectual disabilities.
  • Ensure accurate evaluations and recommendations based on clinical assessments and relevant records.
  • Recommend appropriate levels of care and service needs based on patient evaluations.
  • Complete assessments on time to meet contractual requirements.
  • Manage and supervise the completion of screens for continued stay for individuals in nursing facilities.
  • Ensure compliance with program workflows and standards of practice.
  • Communicate effectively with individuals, families, and healthcare professionals.

Requirements

  • Licensed to practice in Kansas as a Licensed Master's level Psychologist, Licensed Clinical Psychotherapist, Licensed Marriage and Family Therapist, Licensed Clinical Marriage and Family Therapist, Licensed Professional Counselor, Licensed Clinical Professional Counselor, Licensed Social Worker, or Registered Nurse with a specialty in Psychiatric Nursing.
  • Graduation from an accredited college with a relevant degree program.
  • Minimum of 1 year of experience working with individuals with intellectual disabilities or related conditions.
  • Experience with the geriatric population is preferred.

  • In-depth knowledge of medical record organization and medical terminology.
  • Strong clinical assessment capabilities and critical thinking skills.
  • Exceptional verbal and written communication abilities.
