About the position

The Clinical Health Coach position is a full-time, contract role focused on overseeing Remote Patient Monitoring (RPM) and engaging in patient education and coaching. The role is designed for healthcare professionals with an RN or LPN license in Kentucky, working primarily during night shifts. The position aims to improve patient outcomes through a patient-centric approach, utilizing technology and clinical coaching.

Responsibilities

• Manage an active caseload of patients subscribed to the RPM platform or engaged in other telehealth programs.
• Onboard and assist patients in the setup and operation of the mobile app and Bluetooth-enabled devices.
• Utilize the tele-health system and dashboard to interact with and monitor patients.
• Work with patients suffering from chronic diseases such as Prediabetes, Diabetes, Heart Disease, and more.
• Review and evaluate patient behavior and telereadings, providing education and support as needed.
• Communicate daily with patients via text, phone, or video calls regarding their disease state and health education.
• Utilize the electronic health record (EHR) to track patient data and manage schedules.
• Set personal tasks and calendar reminders for patient-specific issues and coordinate with other Clinical Coaches for coverage.
• Participate in the company's online patient community and moderate patient interactions.
• Engage in ongoing training to improve health coaching skills.

Requirements

• Must possess RN or LPN licensure in Kentucky.
• At least two years of experience working in a clinical setting.
• Understanding of remote patient monitoring, tele-health, and chronic disease management.
• Two years of experience as a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
