About the position

The Clinical Manager for Population Health will support the Executive Director in achieving strategic goals related to value-based initiatives. This role involves supervising and coordinating efforts to provide high-quality, cost-effective care to patients while working towards shared savings in value-based payment models.

Responsibilities

• Assist the Executive Director in strategic planning for value-based initiatives.
• Supervise and coordinate actions to deliver comprehensive care to patients.
• Ensure high quality and low-cost care delivery in line with population health goals.
• Monitor and analyze data related to HEDIS and CMS quality measures.
• Collaborate with clinical teams to improve care coordination and ambulatory quality.

Requirements

• Bachelor's Degree in nursing, pharmacy, or other clinical degree.
• 3-5 years of management experience in a healthcare setting, particularly in care coordination or clinic practice.
• Experience in population health or value-based care is preferred.
• Familiarity with HEDIS and CMS quality measures and data analysis.
