About the position

The Registered Nurse - Clinical Nurse Coordinator at Fred Hutchinson Cancer Center plays a vital role in providing patient-centered care as part of an interdisciplinary team. This position is responsible for nursing assessment, diagnosis, planning, implementation, and evaluation, focusing on the physical, emotional, social, and spiritual needs of patients. The Clinical Nurse Coordinator will educate patients and families, coordinate care with various clinical resources, and integrate research into clinical practice, all while ensuring continuity of care and supporting the overall mission of the organization.

Responsibilities

  • Assesses physical, emotional, social, and spiritual needs and evaluates patients' adaptation to health changes
  • Develops a plan of care based on patients' disease, symptoms, and response to treatment
  • Educates patients, families, and caregivers on disease processes, treatment, side effects and symptom management
  • Coordinates with inter-disciplinary team to ensure continuity of care
  • Refers patients, families and caregivers to other clinical resources/professionals as appropriate
  • Implements, evaluates and modifies plan of care
  • Integrates research and evidence-based knowledge into clinical practice
  • Provides direct patient care including education on chemotherapy, central lines, injections, and home care
  • Conducts telephone and email symptom triage
  • Performs dressing changes, medication administration, and injections
  • Facilitates new patient orientation and caregiver support
  • Assesses needs for additional supportive care resources
  • Handles prescription refills and result reviews
  • Coordinates supportive care referrals and paperwork for FMLA and disability
  • Ensures continuity of care with outside providers
  • Collaborates with research team for patients on trial
  • Attends regular team meetings and coordinates care with ancillary services
  • Delegates tasks to appropriate team members and coordinates patient schedules
  • Manages and escalates issues in clinic as needed
  • Documents all nursing care activities accurately and in a timely fashion
  • Participates in continuing education and quality improvement initiatives

Requirements

  • Graduate from an accredited school of nursing
  • Current Washington State Registered Nurse (RN) license or multistate RN license
  • Current BLS AHA Health Provider Card (or equivalent)
  • Critical thinking and clinical judgment skills
  • Time management skills
  • Effective communication skills
  • Ability to listen and provide information clearly

Nice-to-haves

  • BSN prepared nurse
  • OCN (Oncology Certified Nurse) or BMT RN
  • 2 years of oncology nursing experience
  • Previous telephone triage experience
  • Previous outpatient clinic experience
  • Good communication skills
  • Skilled at multitasking and organization
