About the position
The Clinical Nurse Navigator (CNN) position at The University of Kansas Health System is designed to support a new coordinated care model called RETAINWORKS, aimed at helping individuals return to work after illness or injury. This remote role involves collaborating with healthcare providers and patients to facilitate recovery and ensure effective communication and coordination of services. The CNN will oversee the day-to-day operations of the RETAINWORKS program, working primarily with ambulatory care providers and local workforce centers to assist patients in their return to work journey.
Responsibilities
- Coordinate RETAINWORKS patients' progression through defined early interventions to align with treatment goals.
- Communicate timely, relevant, and accurate information to all parties involved in the patient's care.
- Monitor the patient's progression towards desired outcomes.
- Manage the care of the patient population, developing expected patient outcomes and resource utilization.
- Travel within assigned geography to participate in patient encounters and support their success.
- Provide nursing support and coordination to optimize patient outcomes and improve quality of life.
- Advocate for patients through weekly check-ins, identifying their healthcare needs.
- Utilize electronic tools to document assessments and treatment plans efficiently.
- Observe, record, and report patient's condition and reactions to treatments.
- Practice effective time management and organization skills for documentation and follow-up.
- Educate patients and families about health maintenance and diagnostic procedures.
Requirements
- Associate Degree in Nursing
- 2 or more years of RN or LPN experience
- Basic Cardiac Life Support (BLS or BCLS) certification from the American Heart Association within 14 days
- Registered Nurse or Licensed Practical Nurse licensed in the state of work with proof of compact licensure.
