About the position

The Clinical Nursing Supervisor (RN) at PlanMerica Foundation Corp is responsible for overseeing home health care services, combining nursing expertise with managerial skills. This role involves planning, organizing, and evaluating consumer care activities while managing a team of caregivers and clinical support staff. The position requires effective communication and leadership to ensure high-quality care and compliance with healthcare regulations.

Responsibilities

  • Planning, organizing, directing and evaluating consumer care activities
  • Drafting, modifying, and managing consumer care plans and ADL/IADLs
  • Managing caregivers, consumer care oversight, home audits, and consumer QA visits
  • Coordinating with case/care managers, private-pay families, and managing authorizations
  • Monitoring, tracking, and documenting risk management activities
  • Participating in community relations events and marketing campaigns for the agency
  • Planning, facilitating, and managing all caregiver clinical training and continuing education requirements
  • Protecting client and organization privacy and confidentiality
  • Adhering to all PlanMerica, local, state, and federal policies and regulations, including HIPAA
  • Working alternating on-call schedule
  • Performing other duties as assigned

Requirements

  • Graduate of an accredited school of nursing
  • At least 2 years of experience in general nursing, with one year of home health experience preferred
  • A minimum of one year of demonstrated supervisory healthcare-related experience
  • Active Nursing license in Indiana and/or hold a Compact license
  • Knowledge of Medicare, Medicaid, and Commercial payer home health guidelines
  • Ability to communicate effectively, both verbally and in writing
  • Current driver's license and active automobile liability insurance
  • CPR and First Aid certification
  • Pass background check, medical and drug screening
  • Experience with computer and MS Office Suite of applications

Nice-to-haves

  • GA and/or OH nursing licenses
  • Experience in community relations and marketing campaigns
