About the position
The Clinical Pharmacist at Reynolds Memorial Hospital is responsible for providing comprehensive pharmaceutical care through direct patient interaction, operational duties, educational activities, and administrative responsibilities. This role involves leadership within the pharmacy profession and ensures the safe and effective use of medications for patients.
Responsibilities
- Provides leadership and care for patients in assigned areas of responsibility.
- Guides providers and patients on best practices and established guidelines for care.
- Assures safe and effective use of drugs through clinical involvement.
- Successfully completes annual clinical competency evaluations.
- Monitors drug therapy of patients by participating in medical rounds or evaluating medication regimens.
- Participates in the management of acute and chronic disease states, medical emergencies, and adverse drug reactions.
- Communicates relevant findings and recommendations and documents them in the clinical information system.
- Detects, reports, and helps to minimize or manage adverse drug reactions and medication errors.
- Assists nursing and other departments in admission and discharge planning and patient education regarding medication use.
- Responsible for drug use control in assigned service areas.
- Makes decisions related to the provision of pharmaceutical services.
- Dispenses medications from the provider's order and verifies order accuracy.
Requirements
- Bachelor of Science in Pharmacy or Doctor of Pharmacy (Pharm.D.) from an ACPE accredited school of pharmacy.
- Current license to practice pharmacy in West Virginia or license eligible, with completion within 90 days of hire.
- Certification in Basic Life Support within 60 days of hire.
Nice-to-haves
- PharmD from an accredited school of pharmacy.
- Residency training or three years of pharmacist experience.
