About the position

The Clinical Pharmacist at the Northern Arizona VA Healthcare System (NAVAHCS) is responsible for providing comprehensive pharmaceutical care to optimize patient outcomes and resource utilization. This role involves direct and indirect patient care, drug therapy monitoring, and patient education, ensuring safe and effective medication management across various healthcare settings within the VA system.

Responsibilities

  • Monitor therapy on an ongoing basis to assess therapeutic outcomes, adverse effects, and compliance.
  • Analyze laboratory data to rule out drug interferences.
  • Adjust or enter medications by discontinuing and/or generating new/renewed prescriptions.
  • Ensure therapy continuity for patients followed by NAVAHCS and affiliated clinics.
  • Interview patients to determine medication history for potential drug interactions and compliance patterns.
  • Review medication profiles for appropriateness, contraindications, side effects, and patient education needs.
  • Recommend therapeutic alternatives based on patient care assessments and desired outcomes.
  • Participate in medication reconciliation and document clinical intervention findings.
  • Report adverse drug reactions appropriately.
  • Perform all other duties as assigned.

Requirements

  • U.S. Citizenship is required.
  • Graduation from an ACPE accredited College or School of Pharmacy with a Pharm.D. degree or BS Pharmacy.
  • Full, current, and unrestricted license to practice pharmacy in a U.S. state or territory.
  • Proficiency in written and spoken English.
  • Ability to communicate effectively with patients and healthcare staff.
  • Knowledge of pharmacy laws, regulations, and accreditation standards.
