About the position

The Clinical Pharmacist at Oregon Health & Science University (OHSU) plays a crucial role in the Primary Care Clinics, working independently at the highest level of clinical practice. This position is integral to the interdisciplinary healthcare team, focusing on collaborative drug therapy management (CDTM) agreements. The Clinical Pharmacist is responsible for initiating, modifying, or discontinuing medication therapy for various disease states, including diabetes, hypertension, COPD, and smoking cessation. They also serve as consultants for intensive medication therapy management services within their clinics. In this role, the Clinical Pharmacist promotes the economical, safe, and effective use of medication therapy. They maintain comprehensive medication therapy management services, which include conducting physical assessments, ordering necessary lab and diagnostic tests, and referring patients to appropriate healthcare providers. The pharmacist will perform patient triage functions and independently manage medication adjustments and discontinuations. Direct patient care is delivered through various modalities, including face-to-face interactions, telephone consultations, clinical video teleconferencing, shared medical appointments, group education classes, and secured messaging interactions. Documentation of encounters and clinical decisions is essential, as is responding to requests from other healthcare team members. The Clinical Pharmacist will also maintain a collaborative management relationship with the patient's care team and assist providers by approving refill requests as per the pharmacist refill protocol. Additionally, they will provide professional consultative services related to drug information and other healthcare provider inquiries.

Responsibilities

  • Deliver direct care to patients through various modalities such as face-to-face care, telephone, and clinical video teleconferencing.
  • Document encounters and clinical decisions in electronic health records.
  • Respond to requests from other members of the healthcare team.
  • Assist providers by approving refill requests as outlined by the pharmacist refill protocol.
  • Provide professional consultative services related to drug information and other healthcare providers.

Requirements

  • At least 1 year of experience in a clinical pharmacy setting.
  • High School Diploma or GED.
