About the position
The Clinical Pharmacist is a part-time role responsible for executing medication therapy management (MTM) and disease state management through collaborative practice agreements (CPA). This position also involves delivering telehealth pharmacy services and participating in seasonal immunization clinics.
Responsibilities
- Deliver telehealth pharmacy services, including Comprehensive Medication Reviews (CMR), Targeted Medication Reviews (TMR), Medication Reconciliations, and Travel Health Consults.
- Perform other duties as assigned.
Requirements
- Doctor of Pharmacy (PharmD) degree from an ACPE accredited institution
- Registered pharmacist in the State of Ohio with an active status
- Active CPR license (if applicable)
- Immunization training (if applicable)
- Proficient with Microsoft Office Suite
