About the position

The Clinical Pharmacist at Option Care Health is a licensed professional responsible for providing safe and appropriate pharmacy services to patients in home care settings. This entry-level position involves coordinating patient services, monitoring progress, and educating patients and caregivers about medication use, all while adhering to professional standards and regulatory requirements.

Responsibilities

  • Determine the suitability of individual patients for home care.
  • Collect and organize patient-specific information needed for home care suitability.
  • Assess patients' suitability for home care according to Option Care policies.
  • Design, recommend, monitor, and evaluate patient-specific pharmacy care plans.
  • Make recommendations for modifications in prescribed medication therapy when appropriate.
  • Collect and organize information to prevent, detect, and resolve medication-related problems.
  • Complete nutritional assessments and make appropriate recommendations.
  • Modify pharmacy care plans based on evaluation of monitoring data.
  • Provide medication-use education to staff, professionals, and patients.
  • Ensure continuity of pharmaceutical care across different patient-care settings.
  • Prepare and dispense medications following appropriate techniques and policies.
  • Manage medication administration equipment and troubleshoot as needed.
  • Participate in assessing, managing, and reporting medication-related incidents.
  • Respond to drug information requests from healthcare providers and patients.
  • Attend educational seminars to promote professional competency.
  • Utilize pharmacy support personnel effectively and ensure their work accuracy.

Requirements

  • Graduate of an accredited School of Pharmacy.
  • Current, active Pharmacy license from the State Board of Pharmacy.
  • At least one year of experience in sterile IV admixture procedures, knowledge of non-sterile compounding, or completion of an ASHP accredited Pharmacy Residency Program.

Nice-to-haves

  • Completion of an ASHP accredited Pharmacy Residency Program or Advanced Pharmacy Practice Experience.
  • Experience in home care, hospital, or clean room settings.
