Responsibilities

  • Coordinate and oversee clinical pharmacy services
  • Develop and implement clinical protocols and guidelines
  • Conduct medication therapy management and review patient medication regimens

  • Provide education and training to patients and healthcare staff
  • Monitor and evaluate the effectiveness of medication therapies
  • Ensure compliance with regulatory and accreditation standards

  • Manage and mentor pharmacy staff and interns
  • Develop and maintain clinical documentation and records

  • Ensure the safe and effective use of medications
  • Conduct patient assessments and develop individualized care plans

  • Stay current with advancements in clinical pharmacy practice

Requirements

  • 5 year degree in Pharmacy or Pharm.D.
  • Completion of a Pharmacy Residency Program, preferred
  • 3 years Clinical pharmacy experience, preferred
  • Licensed and currently registered in the state of New York to practice Pharmacy, required
