About the position

The Clinical Program Coordinator RN at Swedish Aortic Center is responsible for the comprehensive planning and coordination of patient care within the Swedish Heart & Vascular Institute. This role encompasses managing patient referrals, follow-ups, and care progression, while also ensuring effective integration among patients, staff, and physicians. The coordinator will support the development and implementation of various programs related to aortic disease and facilitate communication with other healthcare providers to optimize patient outcomes.

Responsibilities

  • Plan and coordinate the Swedish Aortic Center (SAC) program throughout the patient experience.
  • Manage patient referral intake, follow-up coordination, and patient care progression.
  • Oversee patient database management and track quality and operational metrics.
  • Serve as a liaison with other programs within Swedish and local/regional hospitals.
  • Assist patients in understanding their diagnosis and treatment goals, including clinical research studies.
  • Handle administrative and operational components of the SAC program.
  • Work with clinical leads and other stakeholders to ensure program needs are met.
  • Facilitate appointments for consults and support services for patient well-being.
  • Track emergency and hospital visits for timely follow-up.
  • Develop evidence-based care approaches to improve clinical outcomes.
  • Support the implementation of the aortic disease pathway and ensure team alignment.
  • Lead SAC program implementation and operational team meetings.
  • Coordinate multidisciplinary planning conferences and document care recommendations.
  • Track and deliver treatment and outcomes data, presenting results quarterly.
  • Establish and maintain positive working relationships with key stakeholders.
  • Accountable for key performance measures of the SAC program.

Requirements

  • Bachelor's Degree in Nursing or a related healthcare field.
  • Washington Registered Nurse License upon hire.
  • National Provider BLS certification from the American Heart Association upon hire.
  • 5 years of professional nursing experience in Cardiology with demonstrated clinical judgment and leadership.

  • Familiarity with aortic disease management and treatment pathways.
  • Strong communication and interpersonal skills.
