About the position

The Clinical Social Work Program Manager at SOME (So Others Might Eat) is responsible for overseeing the Kuehner Place transitional housing program for older adults experiencing homelessness. This role involves managing intake and enrollment processes, providing clinical support to residents and staff, and supervising clinical interns and monitors. The position requires a strong focus on program management, case management, and supervisory responsibilities to ensure the effective delivery of services to vulnerable populations.

Responsibilities

  • Oversee management of waitlist and program intake for Kuehner Place.
  • Assist with social service record collection for new residents ensuring efficiency and compliance with program guidelines.
  • Collaborate with the property management team for timely move-ins and discharges.
  • Obtain program supplies for residents based on program goals and budget.
  • Identify, track, and report program statistics accurately.
  • Develop administrative reports on program statistics.
  • Ensure compliance with guidelines of funding sources and governing bodies for case management.
  • Create and maintain client records including documentation and assessments.
  • Communicate regularly with monitor staff to identify client needs and progress.
  • Contribute to department budget development and administer the budget according to financial reporting requirements.
  • Engage in emergency response planning as needed.
  • Assess senior clients and define barriers to care.
  • Provide resource referrals and develop person-centered care management plans.
  • Represent Kuehner Place as a member of the clinical care team.
  • Implement care interventions and partner with medical staff for chronic health management.
  • Develop case notes and provide status updates on residents.
  • Assist residents in obtaining permanent housing and managing funds for housing.
  • Participate in senior services care team meetings and training.
  • Manage resident issues and provide mediation and coaching to staff.

Requirements

  • Master of Social Work (MSW) and licensed in the District (LGSW or LICSW).
  • 2+ years of previous case management experience with senior and/or at-risk populations.
  • Knowledge of care issues and resources related to aging and vulnerable populations.
  • Problem-solving skills and customer service skills.
  • Excellent verbal and written communication skills.
  • Proficient with MS Office, including Word, Excel, and Outlook.
  • Organized with attention to detail.
  • Ability to work independently and manage time effectively.
  • Supervisory skills and experience.

Nice-to-haves

  • 5-7 years of experience providing services to seniors and low-income populations.
  • 2-3 years of case management experience.
  • Previous experience in an interdisciplinary care team.
