About the position

The Clinical Quality Abstracter role at Hill Physicians Medical Group focuses on supporting quality improvement and clinical programs through chart abstraction, data collection, and various administrative tasks. This position plays a crucial role in ensuring that documentation meets quality measure requirements and assists in enhancing patient care initiatives.

Responsibilities

  • Compare medical chart notes with HEDIS/PQA/IHA measure specifications and guidelines to determine if documentation meets requirements for quality measures.
  • Abstract clinical information from paper/electronic medical records and input data accurately into a quality platform to improve measure performance.
  • Support medication reconciliation post-discharge efforts by sending faxes and calling provider offices/facilities for information.
  • Pull and prep weekly appointment Gaps in Care reports for the Virtual Pharmacy team.
  • Obtain medication lists from provider offices for pharmacist review.
  • Assist with scheduling pharmacist consultations.
  • Pull Likelihood of Hospitalization (LOH) reports and share them with provider offices.
  • Directly outreach to patients to assist in closing gaps in care.

Requirements

  • Bachelor's degree or equivalent experience
  • 3+ years of experience in the healthcare industry
  • 1+ years of clinical chart review/data abstraction experience preferred
  • Basic electronic medical record knowledge and ability to quickly learn new software
  • Ability to organize tasks and complete projects in a timely manner
  • Knowledge of quality measures (i.e., HEDIS)
  • Excellent interpersonal, verbal and written communication skills
  • Attention to detail and ability to follow precise instructions
