Responsibilities

• Provide complete bio-psycho-social assessments based on patient presentation and CMS regulations.
• Participate in the care planning process and facilitate patient engagement in the Plan of Care.
• Deliver direct clinical services tailored to individual patient needs, preferences, and resources.
• Offer case management services to maximize patient eligibility for entitlements and community resources.
• Document all activities related to assessment, clinical work, treatment planning, and crisis intervention according to policy and legal guidelines.
• Maintain knowledge of relevant resources, laws, and guidelines affecting clinical social work practice in Virginia.
• Engage in professional development and maintain necessary licenses and certifications.
• Assess personal practice and seek feedback for improvement, participating in training as needed.

Requirements

• Masters degree in Social Work from a CSWE accredited program required.
