About the position

The Clinical Social Worker at PeaceHealth is responsible for coordinating patient care and treatment plans, focusing on psychiatric, chemical dependency, and psychosocial needs. This part-time position involves assessment, case management, and collaboration with a multidisciplinary team to ensure continuity of care and compliance with legal and ethical standards.

Responsibilities

  • Coordinate the development and implementation of patient care/treatment plans.
  • Conduct intake documentation and ongoing assessments.
  • Develop treatment plans in collaboration with the multidisciplinary treatment team.
  • Manage complex cases and maintain knowledge of community resources.
  • Monitor patient needs and progress, developing aftercare plans.
  • Provide brief therapeutic interventions with patients and families.
  • Participate in community staffings in coordination with the physician and treatment team.
  • Ensure compliance with privacy and confidentiality laws.
  • Interface with legal entities to address treatment needs.
  • Assist in monitoring and evaluating services for quality improvement.
  • Maintain understanding of cultural, ethnic, gender identity, age, sexual orientation, and religion issues.
  • Provide information, education, and direction in response to community requests.
  • Access pharmaceutical and community resources for psychiatric medications.
  • Provide instruction, training, and supervision for graduate-level social work students.
  • Diagnose, assess, and address mental health and chemical dependency conditions.
  • Perform risk assessments for suicidality, homicidality, and grave disability.
  • Screen and identify patients needing care management per high-risk criteria.
  • Collaborate with the interdisciplinary team to develop and monitor discharge plans.
  • Perform other duties as assigned.

Requirements

  • Master's Degree in Social Work from an accredited graduate school of social work.
  • 2 years of clinical experience with designated population.
  • LICSW or LSWAIC status required.

Nice-to-haves

  • Experience in a hospital, emergency room, or community agency setting.
  • Knowledge of community resources, Medicare, Medicaid, and insurance requirements related to continuing care.
