About the position

Under general supervision, provides psycho-social services including psychosocial assessment and interventions, discharge planning, coordination of services, and resource referral.

Responsibilities

  • Consults and follows patients in a community based Palliative Medicine program as assigned.
  • Meets with family members and significant others to obtain a relevant psycho-social assessment and to assess current and potential medical and social drivers of health needs.
  • Develops and implements a plan of care appropriate to identified problems. Shares resultant findings with the appropriate health care team members.
  • Provides counseling services including individual, family and group modalities and other recognized psycho-social therapies in assisting patients and family members.
  • Provides interpretation of the patient's, significant others, and family member's behavior as needed.
  • Provides and facilitates timely completion of appropriate documentation for patient records.
  • Assists team to coordinate referrals, appropriate levels of care, and the use of therapeutic community resources.
  • Provides as well as attends educational in-services for community organizations and providers on palliative and hospice care.
  • Participates actively in problem solving to enhance the most efficient and appropriate plan of care.
  • Coordinates with other Gilchrist Programs to build/develop a complete Palliative Medicine service line.
  • Participates in palliative medicine program development in collaboration with the palliative medicine interdisciplinary team and other service lines.
  • Other duties as assigned.

Requirements

  • Master's Degree in Social Work; Preferred Clinical track.
  • 3 years of clinical social work experience, preferably in a medical setting.
  • Knowledge of various social, home care, extended care, hospice, hospital government program, commercial insurance, and community services.
  • Knowledge of social work theories, therapies, and techniques of casework process.
  • Knowledge of principles and regulations of public welfare and Medicaid.
  • Skill in coordinating efforts of a multi-disciplinary team (agency, hospital, hospice, payer, etc.).
  • Skill in needs analysis.
  • Ability to develop associated treatment modalities.
  • Customer service skills.
  • Skill in word processing programs.
  • Skilled in grief and bereavement.
  • Able to work independently with minimal oversight.

Nice-to-haves

  • LCSW-C preferred.
  • Advanced Palliative Hospice Social Worker Certified (APHSW-C) preferred.
  • Center for Advancing Palliative Care (CAPC) competency - within 90 days of employment.

Benefits

  • Pay Range: $51,351.55 - $79,594.90.
  • Final salary offer will be based on the candidate's qualifications, education, experience and alignment with our organizational needs.
