About The Position

The Clinical Specialist at Haven Hospice plays a crucial role in delivering professional nursing care within the hospice setting. This position is responsible for managing patient workflows, ensuring compliance with policies and regulations, and maintaining effective communication with referral sources to facilitate smooth admissions and transitions in care.

Requirements

• Associate degree in Nursing with current RN license.
• Three years of practice as a Registered Nurse.
• Previous hospice experience preferred.
• Knowledge of the Hospice Philosophy of Care.
• Ability to use Microsoft Word, Excel, Outlook, and other software programs.

Responsibilities

• Provides clinical expertise in the nursing management of patients, documentation, and care planning.
• Processes start of care workflow once the start of care visit has been completed.
• Participates in IDG Reviews visits plotted on SOC or Re-cert visit and adjusts as necessary.
• Maintains orders in HCHB.
• Manages admissions, deaths, discharges, and assists with these to ensure proper documentation is completed.
• Assists in data collection for compliance, quality assurance, implementation of nursing policies and procedures, and staff development as assigned.
• Upholds effective communication with referral sources to ensure smooth transition of admissions.
