About the position

The Clinical Supervisor for Nursing at 21st Century Home Health Services is a leadership role responsible for overseeing the delivery of patient care in home health settings. The role requires a combination of fieldwork and office-based responsibilities, with a focus on improving clinical outcomes and maintaining high standards of patient care.

Responsibilities

• Ensure patient care is coordinated and managed appropriately.
• Serve as the first point of contact for field clinicians for support.
• Oversee clinicians providing patient care in their residences.
• Handle incoming calls and follow up to meet customer needs.
• Coordinate training and educational programs for nurses in the field.
• Assess staff learning needs and formulate specific plans for evaluations.
• Conduct bi-weekly case conferences to discuss patient progress and revise care plans as needed.
• Review and evaluate each case, providing guidance to clinicians.
• Assist clinicians in establishing therapeutic goals and developing care plans.

Requirements

• Two years of management or supervisory experience in a healthcare setting, preferably in Home Health.
• Graduate of an accredited school of nursing; Bachelor's Degree preferred.
• Current CA Registered Nurse License (RN).
• Current Basic Life Support (BLS) certification.
• Valid CA Driver's License with an acceptable driving record.
