About the position
The Clinical Supervisor at Health Link Home Health and Hospice is responsible for overseeing patient care and providing guidance to Clinical Coordinators in a collaborative environment. This role requires strong organizational and communication skills to manage patient care plans and ensure compliance with clinical policies. The ideal candidate will be a dynamic team player with experience in home health, ready to contribute to a supportive work culture.
Responsibilities
- Directly supervise and provide guidance to Clinical Coordinators.
- Provide support and assistance to staff, addressing questions and offering direction to maintain best practices in patient care.
- Oversee patient care plans, ensuring they are tailored to each patient's needs and revised as necessary.
- Answer and triage patient phone calls to the office during business hours, prioritizing clinical concerns.
- Implement and enforce clinical policies and procedures in alignment with industry standards.
- Ensure timely and accurate completion of patient documentation.
Requirements
- Current California RN or LVN License
- Experience working in Home Health
- Strong verbal, written, and interpersonal communication skills
- Familiarity with company-provided Apple products
- Time management and organizational skills
- Attention to detail and accuracy in scheduling tasks
- Ability to function with minimal direction
- Knowledge of medical terminology
- Strong computer skills
Nice-to-haves
- BLS Certification (Preferred)
- Associate Degree (Preferred)
- Customer service-oriented mindset with a focus on patient satisfaction
