About The Position
Rivers Health is looking for a full-time Chronic Care Management/Population Health Certified Medical Assistant (CMA) to coordinate and facilitate the implementation of assessments, care plans, and interventions for a designated patient population. This role focuses on understanding patient health and social situations, including identifying risk factors and social determinants of health.
