About the position

The Clinical Team Lead at Otterbein SeniorLife plays a crucial role in supporting the operational and clinical divisions of the home health agency. This position is responsible for overseeing patient care services, coordinating clinical staff, and ensuring compliance with Medicare/Medicaid standards. The Clinical Team Lead assists in making patient assignments, monitoring care plans, and participating in quality improvement initiatives, all while providing leadership and support to nursing and therapy staff.

Responsibilities

  • Provides support, supervision, and consultation to the clinical staff.
  • Clinically verifies patient care by making patient and personnel assignments and coordinating patient care.
  • Assures that patient needs are continually assessed and that individualized plans of care are developed and updated.
  • Tracks patient cases that are high risk for hospitalization and coaches field clinicians on interventions to prevent hospitalizations.
  • Monitors clinical documentation and ensures timely completion of required documentation.
  • Participates in performance improvement activities including Outcomes Based Quality Indicators (OBQI).
  • Performs quarterly chart audits and utilizes clinical knowledge to oversee patient care.
  • Assists with the orientation of nursing and home health aide personnel.
  • Communicates with billing personnel regarding patient documentation and billing readiness.
  • Maintains current knowledge of home health principles and practices.

Requirements

  • Licensed registered nurse authorized to practice in the state of Ohio.
  • Graduate of an accredited nursing program with a Bachelor's or Master's degree in nursing or related field.
  • Minimum of three years' clinical experience, with at least one year in home health required.
  • Two years in a supervisory position preferred.
  • Demonstrates basic keyboarding and computer skills and is able to use agency software.
  • Possesses leadership skills and enjoys working with people.

Nice-to-haves

  • Experience in memory care settings.
  • Advanced certifications in nursing or home health.
