About the position

The Clinical Triage Nurse (RN) at Yale New Haven Health plays a crucial role in a fast-paced call center environment, utilizing a patient-centered approach to direct patients to the appropriate level of care based on their clinical needs. This position involves effective communication with patients, families, and healthcare providers, ensuring timely and accurate triage while adhering to established clinical protocols. The nurse will also be responsible for mentoring staff and maintaining relationships with clinical practices and community resources.

Responsibilities

  • Performs telephonic triage to a diverse patient population using best practice clinical protocols.
  • Collaborates with healthcare clinicians and resources to secure time-sensitive responses to patients.
  • Uses effective communication skills to assess individual needs and facilitate information exchange.
  • Follows up with patients providing test results, education, and triage for medical management of symptoms.
  • Applies clinical assessment and communication skills while adhering to established clinical guidelines.
  • Follows established call management guidelines and performs accurate documentation for all clinical activity.
  • Responds flexibly to changing situations and reprioritizes work as necessary.
  • Identifies needs and makes recommendations for quality improvement.
  • Maintains strict patient confidentiality.
  • Expands knowledge of community resources to assist patients in accessing appropriate services.
  • Works independently and maintains competence in nursing care.
  • Obtains all requested clinical documentation.
  • Coaches, trains, and mentors new and existing triage staff.
  • Demonstrates patient service excellence at all times.
  • Builds and maintains positive working relationships with patients and clinical teams.
  • Performs quality assessments to ensure proficiency in triage nursing skills.

Requirements

  • Graduate of an accredited school of Nursing.
  • Registered Nurse (RN) license in the state of Connecticut required.
  • Basic Life Support (BLS) certification and Advanced Cardiopulmonary Life Support (ACLS) certification required.
  • 3 years of clinical experience in direct patient care required.
  • Triage or ICU/ED experience preferred.
  • Bachelor's or master's degree in nursing preferred.
  • Excellent interpersonal and customer service skills.
  • Strong ability to educate and excellent communication skills, both written and verbal.
  • Basic computer skills and proficiency in Microsoft Word, Excel, PowerPoint, and Outlook.

Nice-to-haves

  • Experience in triage nursing or emergency/critical care settings.
  • Master's degree in nursing.
