About the position

The Certified Nursing Assistant (CNA) in Hospice at Grane Home Health & Hospice plays a crucial role in providing personal care services to patients in a home setting. This full-time position involves working Monday to Friday during daylight hours, focusing on meeting the individual needs of patients and their families while ensuring a respectful and ethical approach to care. Responsibilities

  • Performs personal care needs when given a patient's assignment and attends to requests promptly.
  • Reports to the Case Manager immediately if unable to perform a certain task.
  • Provides patient and family with positive communication techniques, ensuring confidentiality and ethical care.
  • Meets safety needs of patients and uses equipment safely and properly.
  • Gives personal care including baths, back rubs, oral hygiene, shampoos, and changing bed linen as assigned.
  • Assists in dressing and undressing patients as assigned.
  • Plans and prepares nutritious meals, including shopping, as assigned.
  • Assists in feeding the patient as assigned.
  • Takes and records vital signs as ordered, including temperatures, pulse, respiration, and blood pressure.
  • Provides proper care and observation of patient's skin to prevent breakdown of tissue.
  • Reports on patient's condition and significant changes to the Case Manager.
  • Assists in ambulation and exercise as instructed by the hospice nurse or therapist.
  • Provides normal range of motion and transfers/positioning as directed.
  • Assists with self-administered medications as allowed by state regulations.
  • Offers and assists with bedpans and urinals.
  • Provides assistance with light laundry needs as assigned.
  • Performs range of motion and other simple procedures as ordered.
  • Provides respite for patient's family/caregiver when on-site, as appropriate.
  • Keeps patient's living area clean and orderly as assigned.
  • Adheres to the organization's documentation and care procedures and standards of conduct.
  • Participates in quality assessment performance improvement teams and activities.

Requirements

  • Meets the training requirements of the State.
  • Completed a minimum of seventy-five hours of classroom and supervised practical training, with sixteen hours of classroom completed prior to practical training.
  • Minimum of six months experience providing personal care.
  • At least eighteen years of age.
  • Ability to read and follow written instructions and document care given.
  • Understands hospice philosophy and is comfortable providing specialized care to the terminally ill.
  • Satisfactory references from previous employers.
  • Ability to work with little direct supervision.
  • Secure with issues of death/dying and provides a calm manner in a patient's home.
  • Possess and maintain current CPR certification.
  • Demonstrates tact, patience, and good personal hygiene.
  • Strong oral and written communication skills.
  • Good organizational skills.
