Responsibilities

  • Provides resident care to assigned residents and treats residents with respect at all times.
  • Adheres to facility skin care protocols per policies.
  • Provides personal care such as baths, showers, oral hygiene, skin care, hair care, nail care, monitoring I & O, toileting, etc.
  • Lifts and/or bears weight of residents up to 50 lbs. when transferring.
  • Assists in serving and feeding residents and provides in-between nourishments when appropriate and reports changes.
  • Uses safety measures for all residents and self.
  • Assists with restorative nursing procedures such as range of motion exercises, etc.
  • Responsible for placing clothing in stands and closets.
  • Adheres to facility ambulation/transfers as recommended by therapy.
  • Follows toileting training program established for a resident.
  • Encourages resident to attend all therapies and programs prescribed by Physician/Physician's Assistant and/or recommended by the interdisciplinary team.
  • Answers call lights promptly and provides or obtains the necessary care.
  • Provides daily care based on the current interdisciplinary care plan under the direction of the Charge Nurse and reports changes in condition.
  • Makes beds, cares for soiled linen and resident clothing according to policy and keeps the room neat and safe for resident and staff.
  • Participates in all in-services offered by the facility and puts into practice those theories/techniques presented and taught.
  • Responsible for observing and reporting changes in the physical and emotional norm of residents in their care to the Charge Nurse, and/or Team Leader.
  • Supply hygiene and care products to patients on an as-needed basis.
  • Attend to linen changes and usage of towels, sheets, and wash cloths as needed.
  • Demonstrates good communication skills and makes appropriate responses to residents including the cognitively-impaired residents.
  • Records and documents in electronic medical records under guidance of supervisors and policy and procedures.
  • Reports any suspected mistreatment of residents immediately to supervisor.
  • Lifts objects, including household garbage bags and linen, laundry bags, weighing up to 50 lbs.
  • Assures continued wash/dry/accurate delivery of resident clothing to rooms during shift.
  • Performs duties according to the highest ethical and moral standards and in adherence with the facility's corporate compliance plan and all applicable governing laws.
  • Adheres to the facility's confidentiality statement in accessing and safeguarding protected health information (PHI) of residents and employees, as necessary to perform job duties.
  • Performs any other duties assigned by the Charge Nurse, and/or Team Leader.

Requirements

  • Must be NYS certified
  • Could work as a Companion Care Aide until NYS licensing is secured
  • Experience with activities would be helpful

Benefits

  • Full-time sign-on bonus of $2,500
  • Part-time sign-on bonus of $1,500
  • Pay rate of $16.50 - $22.25 based on experience
