About the position

The CNA-H position is focused on providing routine daily nursing care and services to assigned residents in accordance with their assessment and care plans. The role involves direct interaction with residents, ensuring their comfort and well-being, and maintaining a positive and calm environment within the facility.

Responsibilities

  • Identify residents using wristbands or photo cards before administering treatments or serving meals.
  • Record information using authorized abbreviations established by the facility.
  • Report changes in residents' conditions to the Nurse Supervisor/Charge Nurse promptly.
  • Document entries on flow sheets, notes, and charts in an informative manner.
  • Report accidents and incidents observed during the shift.
  • Maintain confidentiality of residents' protected health information and report any violations.
  • Report unauthorized attempts to access the facility's information system.
  • Perform only nursing care procedures that have been trained for.
  • Prepare residents' rooms for receiving them, ensuring beds are made and admission kits are available.
  • Greet residents and escort them to their rooms, introducing them to roommates and staff as appropriate.
  • Make residents comfortable and inventory their personal possessions as instructed.
  • Assist residents with packing personal belongings during transfers or discharges.
  • Transport residents to new rooms or receiving areas as necessary.
  • Create a warm and positive atmosphere throughout the unit and shift.
  • Meet regularly with nursing personnel to identify and address problem areas.
  • Report complaints and grievances made by residents.
  • Participate in nursing report sessions as instructed.
  • Follow established policies regarding exposure to blood/body fluids.
  • Make beds and provide extra covers as requested.
  • Measure and record residents' temperatures, pulses, and respirations as instructed.
  • Weigh and measure residents as instructed.
  • Ensure residents unable to call for help are checked frequently.
  • Answer resident calls promptly and check on their personal care needs regularly.
  • Assist residents with food arrangements, especially those with sight problems, and record their food/fluid intake.

Requirements

  • Completion of a state-approved nursing assistant training program.
  • Current certification as a nursing assistant (CNA).
  • Ability to communicate effectively with residents and staff.
  • Basic understanding of nursing care procedures and protocols.
  • Compassionate and patient demeanor when dealing with residents.

Nice-to-haves

  • Experience in a long-term care facility or similar environment.
  • Knowledge of electronic health records (EHR) systems.
  • Ability to work in a team-oriented environment.
