About the position

Sentara PACE is seeking a full-time Certified Nursing Assistant (CNA) to join their Home Care team in Norfolk, VA. The role involves providing essential patient care to frail seniors, ensuring they receive the necessary support to remain in their homes and communities. The CNA will perform various tasks including personal hygiene, meal preparation, and monitoring vital signs, contributing to the overall well-being of participants in the program.

Responsibilities

• Perform basic and routine patient care, including personal hygiene and toileting.
• Assist with meal preparation and feeding.
• Check vital signs and monitor intake/output.
• Assist patients with ambulation and transfers from/to bed, toilet, and other areas.
• Provide support to participants in the PACE center or in their homes.

Requirements

• Licensed Nursing Assistant with Virginia Department of Health required.
• BLS certification within 90 days of hire.
• High School diploma or GED or graduation from a state-approved Nursing Assistant Program/Trade School required.
• State CNA certification required within 120 days of hire for recent program graduates.
• Minimum of one year of experience working with the frail or elderly population.

Nice-to-haves

• Experience in home care settings.
• Knowledge of geriatric care practices.
