About the position

The Co-Occurring Community Support Specialist II position at BJC HealthCare is a full-time role focused on providing community support services to individuals dealing with mental health issues and substance use disorders. This position is integral to the community outreach efforts, where the specialist will work directly with clients in their homes and in various community settings. The role requires a strong commitment to helping clients access essential resources such as housing and food assistance, thereby promoting recovery and resilience in their lives. The specialist will carry a caseload that may vary, and in-depth training will be provided during the first 90 days to ensure readiness for the responsibilities ahead. The position operates on a Monday to Friday schedule, with no on-call duties, weekends, or holidays required. Local travel is a significant aspect of the job, necessitating the use of a personal vehicle for client transportation. The specialist will be responsible for meeting clients face-to-face regularly, ensuring that they receive the necessary support and guidance. This role is particularly suited for individuals with experience in working with co-occurring clients, as it involves navigating complex cases and providing tailored support based on individual strengths and needs. As part of the BJC Behavioral Health team, the specialist will contribute to a larger mission of serving over 8,000 seriously mentally ill adults and emotionally disturbed children across several counties. The role emphasizes the importance of documentation, mentorship, and crisis support, making it a dynamic and impactful position within the community health framework.

Responsibilities

  • Provides community support services to complex clients, including those on care plans, using treatment plan interventions that result in positive outcomes based on individual strengths and needs.
  • Completes timely documentation of services that clearly describe the need for the service, the intervention provided, the relationship to the treatment plan, the provider of the service, the date, the actual time and setting of the service, and the individual's response to the service.
  • Contacts individuals and/or referral sources following missed appointments to re-engage and promote recovery/resiliency efforts.
  • Supports individuals in crisis situations.
  • Provides mentorship and job shadowing to community support colleagues.

Requirements

  • Bachelor's Degree in Human Services or a related field.
  • Valid Class D (IL) or Class E (MO) driver's license.

Nice-to-haves

  • Experience working with substance use clients is strongly preferred.
  • Ability to learn new things quickly and a strong ability to use technology.
