About the position

The Case Manager at Zepf Center plays a crucial role in providing community support and coordination of services for individuals experiencing serious mental illness. This position is responsible for assessing client needs, developing individualized treatment plans, and ensuring clients receive the necessary support and resources. The Case Manager works closely with clients in various settings, including their homes and community locations, and is part of a multidisciplinary team focused on delivering trauma-informed care.

Responsibilities

  • Assess client strengths and needs and conduct comprehensive care coordination.
  • Contribute to the development of individualized treatment plans.
  • Participate in daily ACT team meetings for planning and prioritizing tasks.
  • Monitor and evaluate client progress toward goal attainment.
  • Review and modify treatment plans as needed.
  • Maintain contact with clients to ensure stability as indicated in the treatment plan.
  • Provide active outreach and crisis intervention as necessary.
  • Link and refer clients to other services and programs as needed.
  • Assist clients in obtaining and retaining applicable entitlements.
  • Advocate for clients and communicate with family members as appropriate.
  • Document service provision and billing information accurately.
  • Provide on-call coverage during evening and weekend hours as required.

Requirements

  • Bachelor's degree in a related field required.
  • At least 2 years of experience working with a severe and persistent mentally ill population.
  • QHMS certification required throughout employment.
  • Valid Driver's License and acceptable driving record.
  • Knowledge of ACT program and ability to implement care within the fidelity model.
  • Demonstrate knowledge of community resources and mental health services.
  • Excellent written and oral communication skills.

Nice-to-haves

  • CDCA certification preferred.
  • Experience in providing psychoeducation groups.
