About the position

The Community Based Case Manager-ACT at Zepf Center is responsible for providing comprehensive community support and coordination of services to individuals experiencing serious mental illness. This full-time position involves assessing client needs, developing individualized treatment plans, and ensuring clients receive the necessary support and resources. The role requires active outreach, crisis intervention, and collaboration with multidisciplinary teams to promote client stability and well-being.

Responsibilities

• Assess client strengths and needs and conduct comprehensive care coordination.
• Contribute to the development of individualized treatment plans.
• Participate in daily ACT team meetings for planning and prioritizing tasks.
• Monitor and evaluate client progress toward goal attainment.
• Review and modify treatment plans as needed.
• Maintain contact with clients as indicated in the treatment plan.
• Provide active outreach and crisis intervention as necessary.
• Link and refer clients to other services and programs as needed.
• Assist clients in obtaining and retaining applicable entitlements.
• Serve as an advocate for clients and communicate with family members as appropriate.
• Accurately document service provision and billing information.
• Provide on-call coverage during evening and weekend hours.

Requirements

• Bachelor's degree in a related field required.
• At least 2 years of experience working with a severe and persistent mentally ill population.
• Valid Driver's License and acceptable driving record.
• QHMS certification required throughout employment.
• Knowledge of ACT program and ability to implement care within the fidelity model.
• Excellent written and oral communication skills.
• Knowledge of community resources and mental health services.

Nice-to-haves

• CDCA certification preferred.
• Experience in providing psychoeducation groups.
