About the position

The Community Based Case Manager is responsible for providing home-based casework services for families involved with the Department of Child Services (DCS). This role includes supervising family visits and assisting in the assessment of children's readiness for adoption while ensuring their overall well-being.

Responsibilities

  • Provide home visits with families and children to improve caretaking and coping skills.
  • Enhance family resilience and support health and nurturing relationships.
  • Ensure the children's physical, mental, emotional, and educational well-being.
  • Provide accurate and timely information to the referring worker.
  • Maintain timely intervention with the family.
  • Facilitate transportation for clients to appointments, visitations, housing searches, court hearings, and community resources.
  • Supervise family visits and monitor parental roles and responses to children's needs.
  • Document the quality of visits as outlined by DCS.
  • Provide supportive services to help children transition from foster homes to adoptive placements.
  • Participate in Child and Family Team Meetings.

Requirements

  • Bachelor's degree in Social Work, Psychology, or Sociology is required.
  • Experience in child abuse and neglect, trauma, child development, and parenting skills is preferred.
  • Valid driver's license and ability to use a private car for transportation.
  • Must meet minimum vehicle insurance levels as per KidsPeace policy.
  • Must be at least 21 years of age.

Nice-to-haves

  • Experience working with emotionally challenged adults and adolescents.
