About the position

The Community-Based Clinician (CBC-MHP) position at Ada S McKinley Community Services Inc. is a full-time role focused on providing community-based clinical case management and therapeutic support services to individuals and families experiencing mental health and/or substance abuse disorders. Under the supervision of the Manager of Behavioral Health Services, the clinician will engage with clients in various settings, primarily in their homes or community environments. This role is essential in ensuring that clients receive comprehensive care tailored to their unique needs, and it involves a combination of individual, family, and group counseling, as well as service linkage to other necessary resources. The clinician will be responsible for conducting initial behavioral health service screenings, gathering background information, and determining the appropriateness of agency services for clients. Working closely with a Qualified Mental Health Professional (QMHP), the clinician will help develop and implement comprehensive treatment plans, ensuring that all services are provided, monitored, and documented according to agency standards. This includes maintaining communication and coordination with schools, hospitals, and other agencies involved in the clients' lives, as well as ensuring compliance with confidentiality rules and regulations. In addition to direct service provision, the clinician will document all services in case notes, participate in treatment planning reviews, and stay informed about client resources and agency procedures. The role also requires the clinician to act as a mandated reporter for child abuse and neglect, providing necessary transportation for clients to therapeutic activities as outlined in their treatment plans. The position may involve after-hours interventions as directed by a QMHP, and the clinician will be expected to perform other duties as needed to support the agency's mission and goals.

Responsibilities

  • Conduct initial behavioral health service screenings and secure background information.
  • Provide information and referral services to determine client needs and appropriateness of agency/program services.
  • Develop comprehensive behavioral health treatment plans under the clinical direction of a QMHP.
  • Ensure all services are provided, monitored, and documented according to agency standards of care.
  • Coordinate services with schools, hospitals, primary healthcare providers, and other agencies involved in the client's life.
  • Obtain and include all required releases and exchanges of information in the service plan.
  • Establish working relationships with educational, medical, social, and recreational services to meet client needs.
  • Document service provision in case notes according to agency standards.
  • Participate in treatment planning reviews at least every six months.
  • Maintain current knowledge of client resources and agency procedures, including mental health diagnosis and reporting requirements.
  • Act as a mandated reporter of child abuse and neglect as per the Abuse and Neglect Reporting Act.
  • Transport clients to therapeutic activities included in the treatment plan.
  • Perform other duties as assigned.

Requirements

  • Minimum of a bachelor's degree in a behavioral health discipline (social work, psychology, or counseling).
  • 3 years of experience in a related field or in the process of completing a master's degree in behavioral health disciplines.
  • Licensed practical nurse license pursuant to the Illinois Nursing Act of 1987 is also acceptable.
  • Valid driver's license and acceptable Motor Vehicle Report.
  • Verification of insurance coverage for personal auto use.
