About the position

The Community Care Coordinator (Case Manager) role at Hanger, Inc. involves providing guidance and support to patients undergoing amputations, serving as the primary contact for new amputee patients throughout their care journey. Responsibilities

  • Assist patients with amputations by providing information, resources, and support throughout their care process.
  • Coordinate care for new amputee patients from their initial visit through their treatment and recovery.
  • Build and maintain relationships with referral sources and patients to facilitate care coordination.
  • Ensure timely follow-up communications and appointments to keep patients engaged in their care.
  • Act as a liaison between patients, families, and healthcare providers to coordinate pre-operative and post-operative planning.
  • Utilize clinical communication and tracking tools to maintain effective communication with patients and healthcare professionals.
  • Maintain accurate patient files and ensure compliance with Hanger's policies and HIPAA regulations.
  • Process referrals, authorizations, and prescriptions to ensure timely information flow.
  • Provide educational materials and connect patients with community resources and support groups.

Requirements

  • Minimum 2 years of experience in a patient care environment.
  • Bachelor's degree in a health-related field or equivalent work experience.
  • Strong interpersonal, oral, and written communication skills.
  • Excellent organizational and time management skills.
  • Understanding of insurance processes and requirements.

Nice-to-haves

  • Experience in providing high-level customer service in a fast-paced environment.
  • Some clinical background is beneficial but can be learned on the job.
