About the position

The Community Care Nurse at ChenMed plays a pivotal role in enhancing the quality of patient management and satisfaction. This position is designed to promote continuity of care and cost-effectiveness through the integration of case management, utilization review, and discharge planning. The nurse will adhere to strict departmental goals and objectives, ensuring compliance with regulatory standards and policies while delivering quality patient care. The role requires a compassionate and innovative approach to healthcare, as the nurse will be responsible for coordinating the integration of social services and case management functions into patient care, discharge, and home planning processes. This involves collaboration with various departments, external service organizations, and healthcare facilities to ensure a seamless transition for patients. In addition to coordinating care, the Community Care Nurse will conduct concurrent medical record reviews, acting as a patient advocate by investigating and reporting adverse occurrences. The nurse will also educate staff on resource utilization, discharge planning, and the psychosocial aspects of healthcare delivery. Effective communication with physicians is essential, as the nurse will assist in maintaining appropriate costs and desired patient outcomes. The role also includes evaluating patient satisfaction and the quality of care provided, ensuring that clinical resources are utilized effectively to achieve desired outcomes within specific timeframes. The nurse will complete expanded assessments of patients and their families at the time of admission, including psychosocial assessments, and will refer cases to social workers when necessary. By facilitating interdisciplinary patient care rounds and conferences, the nurse will help optimize resource utilization and provide family education on post-hospital needs. The Community Care Nurse will also direct and participate in the development and implementation of patient care policies and protocols, providing guidance in handling special cases or patient needs. This position is dynamic and may involve other duties as assigned by management, emphasizing the importance of adaptability and teamwork in delivering exceptional patient care.

Responsibilities

  • Coordinates the integration of social services/case management functions into patient care, discharge, and home planning processes.
  • Conducts concurrent medical record reviews using specific indicators and criteria approved by medical staff.
  • Acts as a patient advocate by investigating and reporting adverse occurrences and performing staff education related to resource utilization and discharge planning.
  • Promotes effective and efficient utilization of clinical resources to achieve desired clinical outcomes within specific timeframes.
  • Conducts reviews for appropriate utilization of services from admission through discharge.
  • Evaluates patient satisfaction and quality of care provided.
  • Communicates with physicians regularly throughout hospitalization to develop effective working relationships.
  • Assists physicians in maintaining appropriate costs, case management, and desired patient outcomes.
  • Coordinates the provision of social services to patients and families to help them deal with the impact of illness.
  • Completes expanded assessments of patients and family needs at the time of admission, including psychosocial assessments.
  • Refers cases to social workers when patients or families would benefit from counseling for complex discharge plans.
  • Facilitates interdisciplinary patient care rounds and conferences to review treatment goals and optimize resource utilization.
  • Directs and participates in the development and implementation of patient care policies and protocols.

Requirements

  • High school diploma or equivalent required.
  • Bachelor's degree in a healthcare-related field preferred.
  • Valid, active licensure as a professional nurse (RN, LPN, or other nursing discipline) required.
  • A minimum of 7 years' utilization review/case management experience, including psychological aspects of care.
  • A minimum of 7 years' experience in discharge planning from a hospital is preferred.
  • Experience in case management in a hospital and/or healthcare setting highly desired.
  • Possession and maintenance of a current, valid Driver's License required.
  • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within the first 90 days of employment.
