The Community Care Nurse at ChenMed plays a pivotal role in enhancing the quality of patient management and satisfaction. This position is designed to promote continuity of care and cost-effectiveness through the integration of case management, utilization review, and discharge planning. In addition to coordinating care, the Community Care Nurse will conduct concurrent medical record reviews, acting as a patient advocate by investigating and reporting adverse occurrences. The nurse will also educate staff on resource utilization, discharge planning, and the psychosocial aspects of healthcare delivery. Effective communication with physicians is essential, as the nurse will assist in maintaining appropriate costs and desired patient outcomes. The role also includes evaluating patient satisfaction and the quality of care provided, ensuring that clinical resources are utilized effectively to achieve desired outcomes within specific timeframes.
