About the position

Ann's Community is seeking a dedicated Registered Nurse to join our Rehab department at the Irondequoit Campus. This position is integral to ensuring regulatory compliance and fostering a person-centered culture within our facility. As a Care Coordinator, you will play a crucial role in the development and implementation of resident-specific care plans, ensuring that all care provided aligns with established policies and procedures. You will work closely with the Nursing Administration to support departmental leadership and facilitate the ongoing development of staff members. This role requires regular communication with team members regarding any changes in care plans, ensuring that everyone is informed and aligned in their approach to resident care. In this position, you will coordinate daily assignments to ensure that care is delivered effectively and efficiently. You will also collaborate with the Nurse Manager to ensure that clinical operations within the household comply with federal, state, and company policies. Maintaining the confidentiality of resident care information is paramount, and you will oversee the maintenance of clinical records to reflect the current status of residents. This role is not only about providing care but also about mentoring and developing staff, making it a rewarding opportunity for those looking to grow in their nursing careers.

Responsibilities

  • Ensure regulatory compliance and nurture a person-centered culture.
  • Assist Nursing Administration with departmental leadership.

  • Participate in the overall development of staff members in collaboration with the Nurse Manager and other staff.
  • Collaborate with the Nurse Manager to ensure clinical operations comply with federal, state, and company policies.
  • Oversee the maintenance of residents' clinical records, reflecting current status and maintaining confidentiality.

Requirements

  • Three (3) to Five (5) years of related experience/training or equivalent combination of education and experience.
