About the position

The Community Case Manager (Lost To Care) at Vivent Health is responsible for providing intensive, client-centered services to individuals living with HIV/AIDS. The role focuses on expediting access to medical care, addressing barriers to care, and collaborating with various programs and local service providers to enhance the health outcomes of clients. This position emphasizes the delivery of quality patient care within an integrated HIV care and prevention model, aiming for viral suppression and overall stability for clients.

Responsibilities

  • Provide HIV medical case management services in compliance with Federal, State, and agency standards.
  • Utilize the Acuity Index to determine level of need and eligibility for case management services.
  • Perform assessments and develop service plans for clients.
  • Coordinate services and needed care for clients.
  • Maintain appropriate client files and timely documentation of services using electronic health records and case management software.
  • Collaborate with Vivent Health's Pharmacy, Medical, Dental, and Behavioral Health Clinics to ensure timely access to care.
  • Establish and maintain effective working relationships with community resources providing HIV testing and health services.
  • Participate in Food Pantry operations and maintain a functional Vivent Health facility.
  • Update and maintain knowledge of HIV/AIDS, treatments, and related issues through self-study and training.

Requirements

  • Bachelor's degree in social work, health care, or a human service discipline from an accredited college or university.
  • 1 year of experience in HIV/AIDS care or related field.
  • Valid Driver's License and access to a reliable insured vehicle.
  • Proficient in Windows and Microsoft Office.
  • Strong communication skills, both written and verbal.
  • Ability to organize work products and manage time effectively.

Nice-to-haves

  • Experience with Epic software or similar case management software.
  • Knowledge of chronic diseases and mental health issues.
  • Experience working with culturally diverse populations.
