About the position

The Community Based Case Manager plays a crucial role in engaging service consumers (clients) to design and implement a goal-oriented plan that addresses their health, social, and environmental needs. This position is centered around empowering clients to utilize personal and informal resources effectively while linking them to necessary community and formal resources. The Case Manager ensures that clients receive timely access to services, appropriate levels of care, and continuity of care throughout their journey. In this role, the Case Manager will work primarily with individuals residing at the Bucks County Emergency Homeless Shelter, connecting them to essential medical, social, educational, and self-help resources. The Case Manager will coordinate care among various service providers, supporting clients as they work towards recovery and self-sufficiency. This includes assisting individuals with mental health and substance use diagnoses who have faced chronic homelessness, facilitating access to permanent housing without housing readiness requirements. The Case Manager will provide outreach, service coordination, advocacy, and other necessary services in diverse environments, including streets, apartments, shelters, and offices. Utilizing a harm-reduction and trauma-informed approach, the Case Manager will assess Social Determinants of Health (SDOH) and develop service plans to address identified needs. The role also involves completing special assessments as required, enhancing coordination of services for behavioral and physical health, and maintaining professional relationships with community service providers. The Case Manager will adhere to agency policies and procedures, maintain accurate records, and pursue continuing education to enhance their social service knowledge and skills. Overall, the Community Based Case Manager is dedicated to ensuring that clients receive the support they need to achieve their goals and improve their quality of life.

Responsibilities

  • Engage clients in designing and following a goal-oriented plan to address their health, social, and environmental needs.
  • Link individuals at the Bucks County Emergency Homeless Shelter to medical, social, educational, and self-help resources.
  • Coordinate care among multiple service providers to support clients towards recovery and self-sufficiency.
  • Assist individuals with mental health and/or substance use diagnoses in accessing permanent housing without readiness requirements.
  • Provide outreach, service coordination, advocacy, and other needed services in various environments.
  • Utilize a harm-reduction, trauma-informed approach to meet client needs.
  • Participate in Housing First trainings, meetings, and consultations in collaboration with the County.
  • Assess Social Determinants of Health (SDOH) including strengths, risk factors, and health history.
  • Develop service plans to address identified needs in a timely manner.
  • Complete special assessments as required by contractual or program requirements.
  • Assist clients in implementing service plans by linking them to services and monitoring progress.
  • Maintain professional relationships with community service providers and staff.
  • Adhere to agency policies and procedures in carrying out direct service and administrative duties.
  • Complete and maintain orderly, legible, and timely records of services and documents as required by the agency.
  • Pursue continuing education to maintain and upgrade social service knowledge and skills.
  • Attend to the ongoing development of sensitivity to diverse cultural groups.
  • Ensure client privacy and confidentiality as per agency policy.

Requirements

  • Bachelor of Social Work from an accredited school or a bachelor's degree in a closely related health or human service profession.
  • Experience in social services, demonstrated through prior employment, internships, or volunteer service.
  • Proficiency in Microsoft Office Suite.
  • Possession of a valid Pennsylvania operator's license and an insured automobile.
  • Health certification and police clearance for working with children.

Nice-to-haves

  • Experience in supervising or managing case management services.

  • Familiarity with harm-reduction and trauma-informed care approaches.
