About the position

The Community Case Manager at Prairie View, Inc. is responsible for providing comprehensive case management services to children and adolescents, ensuring their needs are met through effective communication and collaboration with families and external care providers. This role involves clinical work, administrative responsibilities, and community education, all aimed at improving patient outcomes and contributing to a positive work environment.

Responsibilities

  • Conduct clinical work to assess and manage patient needs.
  • Handle administrative responsibilities related to case management.
  • Meet billable norms as required by the organization.
  • Transport patients as needed for appointments and services.
  • Provide community education to enhance awareness and understanding of behavioral health issues.
  • Engage in professional development to enhance skills and knowledge.
  • Participate in performance improvement initiatives to enhance service delivery.
  • Contribute positively to the work environment with coworkers and customers.

Requirements

  • Bachelor's degree in a related field of behavioral health sciences or equivalent.
  • Demonstrated skills in the provision of case management.
  • At least 20 years of age.
  • Good listening, verbal, and written communication skills.
  • Ability to observe patient demeanor and nonverbal communication for assessment purposes.
  • Flexibility in scheduling to meet patient and organizational needs.
  • Maintain Kansas State licensure and appropriate CEUs where applicable.
  • Effective communication with referral sources and external care providers.
  • Ability to work effectively with children, adolescents, and their families.
  • Understanding of the special needs and behaviors of children and adolescents ages 2 to 18 (up to 22 years old if on the Waiver).
  • Valid Kansas driver's license and insurable through Prairie View insurance company.
  • Ability to pass physical restraining program.
  • Physical capability to bend, squat, and kneel for activities with children.
  • Proficiency in basic keyboarding/word processing skills.
  • Ability to pass Kansas Bureau of Investigation Screen, Central Registry Screen, and DCF background check.
