About the position

The Community Case Manager at Lutheran Social Services of Central Ohio plays a vital role in supporting victims of domestic violence through the CHOICES program. This position involves conducting initial interviews, assisting clients with housing, and implementing goal-oriented plans to enhance safety and quality of life. The role emphasizes crisis intervention, collaboration with community resources, and fostering supportive relationships for clients.

Responsibilities

  • Receive referrals from CHOICES hotline resource specialist and shelter case management staff and conduct initial interviews with potential clients.
  • Attend all meetings with CHOICES and other partners as needed and requested.
  • Assist residents, CHOICES case management staff, and community partners with securing and preparing their housing for occupancy as quickly as possible.
  • Work closely with clients to develop and implement intentional action-oriented goal plans based on the CTI model, focusing on maintaining housing, enhancing safety, and creating a meaningful quality of life.
  • Implement planning strategies that reflect client strengths and needs, safety of the household, client responsibilities, and action steps to help clients achieve their goals.
  • Provide budgeting and other planning assistance toward the goal of housing retention and safety.
  • Refer clients to partner agencies and community resources that benefit their short- and long-term goals including physical and behavioral health care needs.
  • Assist with crisis intervention and de-escalation by providing and modeling professional and calm problem-solving behavior for clients, colleagues, and the community.
  • Work with clients to establish ongoing support relationships with community resources, family, and friends.
  • Facilitate peer group support for former shelter residents to discuss triggers, challenges, and successes.
  • Back up CHOICES Shelter services staff as needed, including hotline and shelter advocate service area.
  • Participate in all appropriate agency meetings, working with colleagues toward positive client outcomes.
  • Contribute to an atmosphere of safety, respect, dignity, support, and resources through adherence to the CHOICES Code of Ethics and Confidentiality Agreements.

Requirements

  • Bachelor's degree in social work or related field preferred or equivalent experience required.
  • LSW preferred.
  • Experience with Crisis Intervention, Case Management, and Advocacy.
  • Commitment and ability to work as a team with diverse staff, volunteers, and clients; using a Trauma-informed Care approach preferred.
  • Knowledge of domestic violence, community resources, and ability to engage clients experiencing trauma.
  • Knowledge of basic computer skills.
  • Valid Ohio driver's license, access to reliable insured transportation, and ability to meet LSS insurance standards required.

Nice-to-haves

  • Experience in a non-profit environment.
  • Familiarity with local community resources for domestic violence victims.
