About the position

The Community Case Manager at Vivent Health plays a crucial role in providing intensive, client-centered services to individuals living with HIV/AIDS. This position focuses on expediting access to medical care, addressing barriers to care, and collaborating with various programs and local service providers to enhance the well-being of clients. The role emphasizes quality patient care and professional growth within an interdisciplinary team dedicated to serving underserved populations.

Responsibilities

  • Provide HIV medical case management services in compliance with State and agency standards.
  • Administer the Acuity Index to determine eligibility for case management services.
  • Conduct assessments, service planning, and monitoring for clients with HIV infection.
  • Offer consultation, referral, and guidance to HIV-positive individuals to optimize their success in addressing needs.
  • Assist clients in accessing benefits and insurance programs as needed.
  • Provide direct access to short-term programs to stabilize housing circumstances or refer to long-term housing programs.
  • Maintain client files and timely documentation of services using case management software.
  • Participate actively in the Social Services Department, including meetings and training.
  • Collaborate with medical, pharmacy, dental, and behavioral health personnel to ensure timely access to care.
  • Establish relationships with community resources providing HIV testing and health services.
  • Link clients to available services through Vivent Health and the community.
  • Provide education on HIV disease and treatment options to clients and service providers.
  • Administer HUD housing programs and assist with housing stability and health outcomes.
  • Participate in Food Pantry operations as needed.

Requirements

  • Bachelor's or graduate degree in social work, health care, or a human service discipline from an accredited college or university.
  • Minimum of two years' experience offering direct client contact in a community-based social service or health service agency.
  • Extensive knowledge of medical, social, and mental health delivery systems.
  • Valid driver's license and willingness to travel within the designated territory.
  • Effective written and verbal communication skills, including client-centered communication and assessment skills.
  • Ability to establish rapport with culturally diverse populations.
  • Strong organizational skills and ability to manage time effectively.
  • Knowledge of and sensitivity to the HIV/AIDS patient population.

Nice-to-haves

  • Bilingual in Spanish is a plus.
