About the position

The Community Case Worker position at Allied Staff Links involves providing support and coordination for clients transitioning from institutional settings to independent living. The role focuses on ensuring clients receive necessary medical resources, housing arrangements, and community resources while promoting their health and welfare. This position can be performed remotely and requires effective communication and organizational skills.

Responsibilities

  • Call and schedule clients
  • Assign clients to care coordinators on a daily basis
  • Assist with filling out forms and obtaining needed documentation for client transitions
  • Locate and arrange appropriate, accessible housing
  • Work with local housing authorities regarding housing availability and subsidies
  • Arrange the client's physical move from an institutional setting
  • Ensure medical resources are available and medical needs are met
  • Set up the client's new home, including lease documentation and utility hook-ups
  • Connect clients to community resources

Requirements

  • Bachelor's degree in psychology, social work, social services, human services, behavioral sciences, or a closely-related field
  • 1 year of experience in the coordination of independent living services (ILS) or with the aging population
  • Crisis intervention skills
  • Report writing skills
  • Crisis management skills
  • Proficient in Microsoft Office
  • Strong communication skills

Nice-to-haves

  • Associate's degree
