The Community Choices Care Manager is a full-time position (37.5 hours/week) based in the Home Care Department, located at 67 Millbrook Street, Worcester, MA 01606. This role is hybrid, allowing for a combination of remote and in-office work. The primary responsibility of the Community Choices Care Manager is to conduct home visits to assess elders for program eligibility, develop, implement, and monitor individualized care plans, and perform other related duties as assigned. The position requires excellent written, verbal, and interpersonal communication skills, as well as strong organizational abilities. The ability to multi-task and prioritize effectively in a fast-paced environment is essential. The role also involves moderate physical mobility, including the ability to lift and carry objects weighing up to 20 pounds. A hiring bonus of $1,500 is offered, distributed in two parts: half after 120 days of employment and the remaining half after one year of active employment. The position does not require working on holidays or being on-call, and it offers flexible scheduling designed to promote work/home balance.
