About the position

The Community Connections Program Manager (CCPM) at TNC Community is responsible for overseeing and enhancing the Community Connections program, which serves individuals with intellectual and developmental disabilities. This role involves managing service delivery, supervising staff, and ensuring that all services align with Individual Support Plans (ISPs). The CCPM acts as an advocate for individuals, ensuring their needs are met while fostering community connections and maintaining compliance with relevant regulations.

Responsibilities

  • Manage Community Connections programs and services, including day habilitation and employment services.
  • Supervise Community Connections Coordinators and Specialists, ensuring effective service delivery.
  • Attend ISP meetings and maintain relationships with clients and support coordinators.
  • Complete monthly program summaries and submit billing information as required.
  • Track referrals and manage wait lists for services.
  • Develop and implement training services related to programmatic processes.
  • Maintain individual and program confidentiality and uphold individuals' rights.
  • Report suspected abuse and neglect according to TNC policies.
  • Participate in team meetings and complete necessary paperwork in a timely manner.

Requirements

  • Bachelor's degree in Human Services or related field.
  • Minimum 2 years of experience in social services, particularly with individuals with IDD.
  • Strong communication skills, both written and verbal.
  • Experience in supervising staff and managing programs.
  • Ability to clear background checks and drug/alcohol screenings.

Nice-to-haves

  • CPR/First Aid certification
  • Experience with Microsoft Office software
  • Familiarity with CARF Standards and DMH regulations
  • Experience in mentoring and training staff.
