About the position

The Community Connector role is a remote position focused on engaging with newly enrolled VA Medicaid members. The primary responsibilities include making outbound calls to these members, conducting health screenings, and addressing any inquiries they may have. This position requires strong customer service skills and the ability to manage multiple databases while providing support to vulnerable populations in navigating their healthcare needs.

Responsibilities

• Make outbound calls to newly enrolled VA Medicaid members to establish first contact.
• Complete health screenings and answer member questions.
• Assist members in scheduling appointments and arranging transportation for healthcare visits.
• Help members access social services and community resources for housing, food, and employment.
• Conduct research to locate members who have been difficult to contact.
• Participate in ongoing outreach activities, both telephonically and face-to-face.
• Guide members in maintaining Medicaid eligibility and accessing financial resources.

Requirements

• High School Diploma or GED required.
• Minimum of 1 year experience working with underserved or special needs populations.
• Valid driver's license with a good driving record and reliable transportation.

Nice-to-haves

• Associate's Degree in a healthcare-related field preferred.
• Bilingual skills based on community needs.
• Familiarity with healthcare systems is a plus.
