About the position

The Community Connector role is a remote position focused on engaging with newly enrolled VA Medicaid members. The primary responsibilities include making outbound calls to establish first contact, conducting health screenings, and addressing member inquiries. This position requires strong customer service skills and the ability to navigate multiple databases while assisting members in managing their healthcare needs and connecting them with community resources.

Responsibilities

• Make outbound calls to newly enrolled VA Medicaid members to establish first contact.
• Complete health screenings and answer member questions.
• Assist members in navigating and maximizing their health plan benefits.
• Schedule appointments with providers and arrange transportation for healthcare visits.
• Help members access social services such as housing, food, and employment resources.
• Conduct research to locate members who have been difficult to contact.
• Participate in ongoing or project-based activities requiring extensive member outreach, both telephonically and face-to-face.
• Guide members in maintaining Medicaid eligibility and accessing financial resources.

Requirements

• High School Diploma or GED required.
• Minimum 1 year experience working with underserved or special needs populations.
• Valid driver's license with a good driving record and reliable transportation.

Nice-to-haves

• Associate's Degree in a healthcare-related field (e.g., nutrition, counseling, social work).
• Bilingual based on community need.
• Familiarity with healthcare systems.
• Knowledge of community-specific culture.
• Experience with health care basics, community resources, social services, and/or health education.
• Current Community Health Worker (CHW) Certification preferred (for states other than Ohio, Florida, and California).
• Active and unrestricted Medical Assistant Certification.
