Community Habilitation Specialist

About the position

The Community Habilitation Specialist plays a crucial role in supporting developmentally disabled individuals enrolled in the HCBS Waiver program. This position focuses on providing assessment, training, and assistance to enhance the independence and inclusion of individuals in their communities. The specialist will work directly with consumers in their homes or community settings, following the guidelines set forth in their Individualized Service Plans (ISP).

Responsibilities

• Provide assessment, training, and assistance to developmentally disabled individuals.
• Work with consumers in their homes or community based on their Community Habilitation Plan.
• Facilitate individual independence, productivity, and community inclusion.
• Maintain regular communication with supervisors regarding consumer progress and relevant issues.
• Communicate changes in schedules, lateness, and absences to supervisors and consumers.

Requirements

• High School Diploma or GED is required.
• Valid NYS Driver's License is required.
• Experience in direct support is preferred.

Nice-to-haves

• Knowledge of American Sign Language is a plus.
