Community Habilitation Specialist

About the position

The Community Habilitation Specialist plays a crucial role in supporting developmentally disabled individuals enrolled in the HCBS Waiver program. This position focuses on providing assessment, training, and assistance to facilitate independence, productivity, and community inclusion for consumers in their homes or community settings. The specialist is responsible for maintaining communication with supervisors and consumers, ensuring adherence to individualized service plans, and addressing any scheduling or performance issues.

Responsibilities

• Provide assessment, training, and assistance to developmentally disabled individuals.
• Work with consumers in their homes or community based on their Community Habilitation Plan.
• Facilitate independence, individualization, productivity, and inclusion into the community.
• Maintain communication with supervisors regarding consumer progress and job performance.
• Communicate changes in schedules, lateness, and absences to supervisors and consumers.

Requirements

• High School Diploma or GED is required.
• Valid NYS Driver's License is required.
• Experience in direct support is preferred.

Nice-to-haves

• Knowledge of American Sign Language is a plus.
