Community Habilitation Specialist

About the position

Community Habilitation Specialists work one-on-one in the community with individuals who have intellectual and developmental disabilities (I/DD). The role involves providing direct care services, promoting community integration, and supporting individuals in achieving their personal goals. The position offers customizable hours, allowing for flexibility in scheduling, and is focused on enhancing the quality of life for those supported by YAI.

Responsibilities

• Provide direct care services including counseling and training in Activities of Daily Living (ADLs).
• Ensure the health, safety, and wellness of the individuals supported.
• Build and maintain positive relationships with individuals and their families.
• Assist individuals in maximizing their strengths, interests, and abilities through teaching and emotional support.
• Facilitate community integration, recreational activities, and socialization.

Requirements

• High School Diploma or GED.
• Valid Driver's License and use of a personal vehicle (mileage reimbursed).
• Work or life experience in the I/DD field is a plus but not mandatory.
• Ability to complete three to five weekdays of paid New York State-mandated OPWDD training during normal business hours.
