About the position

The Community Health Advocate, Experienced will play a crucial role in supporting Blue Shield of California Promise Health Plan members by conducting outreach and assessments focused on social drivers of health needs. This position requires the advocate to identify resource gaps in both medical and community services, and to assist in the implementation of new services aimed at closing these gaps. The role involves assessing psychosocial and social drivers of health needs for moderate-risk members, documenting assessment results, and augmenting available information in appropriate systems. In addition to assessments, the Community Health Advocate will establish relationships and partnerships with community organizations, health plans, and providers. This includes participating in community engagement activities with local agencies such as faith-based organizations, community centers, government agencies, parks, recreation centers, and schools. The advocate will assist members with moderate health conditions by problem-solving to address barriers, which may involve visiting members at their homes, accompanying them to medical appointments, and helping them complete forms to access necessary services. The role also requires the advocate to actively engage with members and their family support systems, building rapport and facilitating collaborative communication. Identifying social drivers of health concerns and developing plans to address health disparities is a key responsibility, along with documenting member updates and progress notes in appropriate systems. The advocate will also be responsible for filing and following up on mandated reporting, such as Adult Protective Services and Child Protective Services, and performing other duties as required.

Responsibilities

  • Conduct outreach to Blue Shield of California Promise Health Plan members and perform assessments for social drivers of health needs.
  • Identify resource gaps in medical and community services and support the implementation of new services to address these gaps.
  • Assess psychosocial and social drivers of health needs for moderate-risk members and document assessment results.
  • Establish relationships and partner with community organizations, health plans, and providers through community engagement activities.
  • Assist members with moderate health conditions by problem-solving to address barriers and navigating needed community and medical services.
  • Engage and build rapport with members and their family support systems, facilitating collaborative communication.
  • Identify social drivers of health concerns and develop plans to address health disparities in accessing appropriate care and services.
  • Document member updates and progress notes, submit timely reports, and provide recommendations for improved member outcomes.
  • File and follow up on mandated reporting, such as Adult Protective Services and Child Protective Services.
  • Perform other duties as required.

Requirements

  • Bachelor's degree in Social Work, Psychology, Biology, Public Health, Nursing, Community Health, or a related field preferred, or equivalent experience in a related field.
  • 3 years or more of experience working with individuals needing assistance with complex health and social issues.
  • Minimum of 1 year experience navigating, linking to, and interacting with community-based organizations and governmental systems.
  • Valid CA Driver's License and proof of auto insurance coverage required.
  • Proficient skills with computer programs, including Microsoft Excel, Outlook, Word, and PowerPoint preferred.
  • Excellent communication skills, both orally and in writing preferred.
  • Knowledge of health disparities in diverse communities preferred.
  • Effective time management skills and ability to prioritize multiple responsibilities.

Nice-to-haves

  • Experience working with Medi-Cal populations preferred.
  • Bilingual/Spanish speaking preferred.
